Your first quiz date will be the date your year starts, no exceptions.
CEU Network Account Number is your License Number.
Each quiz must have an evaluation form completed. I will mail an evaluation form to you 
once I receive your first quiz.

Make copies and with your second quiz send in 2 evaluation forms completed (one for the 2nd quiz and one for the first quiz already sent in).
